Hazard Assessments & Safety Plans

Cal/OSHA requires employers to conduct hazard assessments of their employees work areas. The Field Safety Plan (FSP) helps you identify critical resources for your trip and potential safety hazard mitigation.

Completing a Field Research Safety - Hazard Assessment Questionnaire (HAQ), this will help determine if there are potential hazards associated with your field work that require a Field Safety Plan.

We will inform you if a Safety Plan is required based on your Field Research Safety - Hazard Assessment Questionnaire.

Submit your completed safety plan for our review 2 weeks prior to your trip via email: Fieldresearch@ehs.ucla.edu(link sends email)

Please allow 2-3 business days for EH&S review.

FS - Safety Plan Template

In the event that an emergency occurs at your destination travel registration allows your campus Risk Management office to know where you are there. Registering your travel also signs you up to receive alerts from UC’s security provider, Crisis24.

There are two ways that travel can be registered:

Booked through UC Travel or Connexxus	SWABIZ bookings or for reservations made outside Connexus
Travel Itinerary is automatically registered with Worldcue. Note: For bookings of two or more people on the same confirmation number, the travel arranger will need to register the travelers via Away.	Register your trip with UC Away

Once registered check your email for your “welcome email” and complete your travel profile.
