A hazardous waste manifest also known as a EPA's Uniformed Waste Manifest is a legally required document to track hazardous waste from generation to disposal. This document ensures accountability and helps prevents illegal dumping.
Any shipment of hazardous waste that is transported off-site, including those for treatment, storage, or disposal.
This includes state-regulated hazardous wastes, RCRA wastes, and Toxic Substances Control Act (TSCA) wastes
This generally excludes household hazardous wastes (HHWs).

- Friable: DTSC classifies asbestos-containing material as hazardous waste if it is “friable” and contains one percent (1.0%) or more asbestos as hazardous waste. California Code of Regulations, section 66261.24. Friable Asbestos must be transported on a Unified Hazardous Waste Manifest.
- Non-Friable: DTSC considers non-friable bulk asbestos-containing waste to be non-hazardous regardless of its asbestos content, so it is not subject to regulation under Title 22, Division 4.5, of the California Code of Regulations. This material be transported for disposal on a non-hazardous manifest.
At the start of your project complete a Asbestos and Lead Project information Form and email asbestosandlead@ehs.ucla.edu prior to coordinating disposal.
Manifest can only be signed at the time of transport. Schedule an appointment with our team at hazardousmaterials@ehs.ucla.edu, allow for 5 business days prior notice to schedule an appointment. Same day request may not be fulfilled.
Manifest can only be signed by individuals designated by EH&S. Those that sign manifest take on the responsibility and liability associated with that waste. By signing a manifest the individual is acknowledging the following:
- The information on the manifest is accurate
- Complies with state and federal laws as it relates to quantities, weights, packaging, labeling, transporting, and disposal

Designees are responsible for:
- Reviewing shipment prior to signing, verifying the following information is correct on the manifest
- EPA ID and address are match
- Shipment quantities, unit of measure, and material description is accurate
- Sign after transporter signs
- Retain the generator's copy for your files
- Email generator's copy to hazardousmaterials@ehs.ucla.edu within 1 business day
- Retain copies on site for at least three years
- Signing Land Disposal Restriction (LDR), if applicable

What is an EPA ID Number?
An EPA ID number is issued by U.S. Environmental Protection Agency (EPA) or Department of Toxic Substance Control (DTSC) for business that generate or handle hazardous waste It's purpose is to track waste from origin to final disposal referred to as "cradle to grave".
UCLA holds several EPA ID numbers for sites that routinely handle hazardous waste.
Sites that do not routinely handle hazardous waste
A temporary EPA ID number can be requested for sites that do not planning on normally handling hazardous waste. These temporary EPA ID numbers are only valid for 90 days. In a project with construction activities identifying the waste being produced and planning for disposal determines what type of EPA ID number is requested and when it is requested.
