Before you begin we recommend you schedule a consultation with our office help identify your needs:
- Completed a Controlled Substance Use Authorization (CSUA)
- We can help you complete the online DEA application via zoom DEA 225 form, schedule a time with use Book a Consult
- If you submit an DEA 225 on your own and alert us of the confirmation number so we will request the DEA expedite your application.
How to complete a 225 Form

Select you "Business Category": Researcher under column labeled Form 225
Select from the dropdown "Business Activity": Researcher (II -V), please do not be concerned about the fee we are taxed exempt.

If you have a representative completing this form on your behalf they will need a Power of Attorney. Which you upload at this point.
If you are completing this application yourself select "Proceed"
You are requesting an individual researcher registration, this means that you are personally responsible for all the information your provide and ensuring compliance with DEA regulations.
- "Additional Company Information": please include UCLA we want the DEA to understand how many registration are at UCLA
- "Business Address": this is the address of where you are planning to store your controlled substances.. UCLA campus zip code is 90095.
- For example you are located in Semel, Room A1-123A.

- "Contact Name": You can enter you lab's primary point of contact here. Please note this should not be someone with no authority in the lab.
- "Mail Address": this is your UCLA mail to address. All postal mail is delivered to MDDS they then distribute the mail to all of campus using a UCLA Box# or mail code.
After completing this section the DEA will send you a token code to validate you email. Enter code and select proceed.
The online application may provide suggested corrected address you do not need to use the suggested address, select proceed to continue.
UC's Tax-Exempt Status Overview
There are no fees associated with this application. The Regents UCLA is a tax-exempt institution all work conducted for the university is covered.
You only need to enter Tax ID: 956006143 and check Certificate for Fee Exemption
Select proceed to continue and screen will expand.

Controlled substances are scheduled 1 through 5, for more information about schedules visit: "What is a controlled substance?"
For this part of the application you will need to select all the schedules that apply for the drugs you are planning to use.
You can look the drug schedule on the DEA Controlled Substance Alphabetic List. PDF. Please note that the letter "N" after the schedule number mean that the controlled substance is not a narcotic.
Schedule 2
If you select a schedule 2 either narcotic or non narcotic also select the check box: "check here if you require order forms to only purchase Schedule I and II from suppliers.
California does not have a state license for researchers, you can leave this blank and select proceed.
You will be ask to answer the following four questions:
- Has the applicant ever been convicted of a crime in connection with controlled substance(s) under state or federal law, or been excluded or directed to be excluded from participation in a medicare or state health care program, or any such action pending?
- Has the applicant ever surrendered (for cause) or had a federal controlled substance registration revoked, suspended, restricted or denied, or is any such action pending?
- Has the applicant ever surrendered (for cause) or had a state professional license or controlled substance registration revoked, suspended, denied, restricted, or placed on probation, or is any such action pending?
- If the applicant is a corporation (other than a corporation whose stock is owned and traded by the public), association, partnership, or pharmacy, has any officer, partner, stockholder or proprietor been convicted of a crime in connection with controlled substance(s) under state or federal law, or ever surrendered or had a federal controlled substance registration revoked, suspended, restricted or denied, or ever had a state professional license or controlled substance registration revoked, suspended, denied, restricted, or placed on probation, or is any such action pending?
This section does not work you can select proceed.

You have arrived to the end of the application which is a summary of all that you have completed. Review the information and correct if necessary. If all the information looks correct complete it by typing you e-Signature and checking of the final check box validating your email.
Once you submit your application you will receive a confirmation number save this page as a PDF for your records. That confirmation number is important as it is a temporary ID number to reference your application on all communications with the DEA until you receive your registration
