PIs are responsible for ensuring that staff and students using controlled substances in conjunction with their research and teaching are listed on their CSUA and comply with associated regulations, policy, and training.
Researchers must register with the Controlled Substance Program prior to the use, purchase or transport of controlled substances at the UCLA campus.
A CSUA number (e.g. CS001) will be issued by the Controlled Substance Program and is associated with a department DEA registration. The CSUA ID number is the unique identifier that is used on all documents and communications.
Are faculty, staff, students, or visiting scholars identified by a Principal Investigator as needing to handle or access Controlled Substances for department approved projects at UCLA. The number of authorized personnel shall be kept to a minimum in order to maintain adequate security.
In order to be an authorized user the following must be completed:
- Complete and submit Personnel Screening Data Sheets (PSDS) for each user, Email signed copy to aleiva@ehs.ucla.edu
- Complete the Controlled Substance Training in WORKSAFE
Additional individuals listed on a CSUA that many not handle material but may be included are:
Primary and Secondary Contacts: all CSUA must have a primary and/or secondary point of contact listed on a CSUA. These individual(s) can be the PI or a designated authorized user. They do not need to handle controlled substances but must be able to answer questions or know who to ask in their lab the event of a inspection by the DEA. They are the point of contact for any updates to a CSUA.
Key Coordinators: key coordinators are individuals listed in space inventory that have the authority to request keys or change locks in specified rooms. This authority requires us to have to list them on a CSUA.
Administrative Staff: administrative staff may be used as primary points of contacts, key or file keeps, or my process orders or receive material on behalf of the lab as a result they would added to a CSUA
Personnel Changes- It is important to be able to identify who are the individuals that have access to controlled substances. Update your CSUA as soon as you have update changes by contacting the Controlled Substance Program. We will remove or add new personnel to your CSUA. Personnel records will be maintained for 3 years after removal from CSUA.
All CSUAs require justification of work for use of controlled substances in your research project in order to be approved. The justification can be the form of a protocol such as an IACUC (ARC or IRB) protocol, BUA, or RUA. Some research work line In- Vitro work may not have an institutional approved protocol, for these a written description must be provided to the Controlled Substance Program.
