Hazard Door Cards

A fire diamond door card is a sign placed on the exterior of the main lab indicating any potential hazards, Personal Protective Equipment (PPE) requirements, entry and exit requirements, and contact information. The fire diamond comes from the National Fire Protection Agency (NFPA) 704 standard which requires a summary of the hazards and severity of these hazards as they relate to emergency response.

In the event of an emergency, first responders can understand what precautions are necessary to take prior to entering the lab. Maintaining an up-to-date and accessible emergency contact is crucial in such situations, given the dynamic nature of research and hazards. Depending on the circumstances, additional information may be required before proceeding.

Rooms with hazardous materials require a door card.

The request form can be found above. The Fire Diamond door card encompasses Biosafety levels in the main requested room. If there is a change in Biosafety levels, there should be an Biosafety door card.

Please be sure to:

  • Have your chemical inventory up-to-date
  • Have your lab's Use Authorizations up-to-date
  • Designate at least two emergency contacts 

That's okay! Our staff can provide you with a temporary door card, please be sure to indicate that your lab is new on the email or form. The door card will state that the hazard assessment may be incomplete it is still important to have your designated contact information, any anticipated specific hazards, and entry and exit requirements.
